Note: 
Migration Queensland will only accept current form versions – these are available in your document portal. 

These are the minimum requirements for documents to support a state nomination application. Migration Queensland has discretion to request further information or supporting documents if required.

Document checklist explanatory notes

  • Passport bio page – good quality colour scan of bio-page of your valid passport. 
  • VEVO statement – Visa Entitlement Verification Online allows a visa holder to check their visa conditions and validity. Please provide a copy of your recent VEVO statement.
  • State nomination application form – required for all applicants. This form is only available when you receive an invitation from Migration Queensland.
  • Skills assessment or Evidence of Registration – a valid skills assessment in your nominated occupation. Some occupations must provide Evidence of Registration instead. Doctors, GPs, Resident Medical Officers and medical specialists will need to provide their current AHPRA registration. Solicitors will need to provide their Certificate of Admission.
  • Evidence of address – provide a copy of your current rental lease or a recent (issued within last three months) utility bill with your name on it. If this cannot be provided, please provide a statutory declaration declaring your current residential address. 
  • Evidence of salary payments – at least three payslips to demonstrate the required period of work experience in Queensland. Payslips should cover the beginning, middle and most recent period of employment. 
  • Payslips used to calculate ROI Salary – provide the two payslips used to calculate your annual salary for your ROI form submission. These payslips must be from the time you submitted your ROI, not your current payslips.
  • Bank statements showing declared funds – bank statements that show the declared amount of settlement funds. The amount declared in the application form and the amount on the bank statements must match. Please see the Settlement Funds Factsheet for more information. 
  • Statement of Service form – this form is to be completed by the employer/s of onshore applicants, to support the work experience requirement. If you have had more than one employer during the period of required work experience, each one will need to complete this form. If you are employed through a labour hire company, your statement of service must come from the placement company. 
  • Letter from employer – offshore applicants must provide reference letters to evidence the minimum period of post-qualification work experience required. You must also provide a letter from your current employer. If these letters are not in English, a translated copy must be provided as well. 
  • Employment contract – a complete employment contract that demonstrates ongoing, full-time (minimum 30 hours per week) employment for the required period, at the time of submitting your EOI.
  • myGov income statement – this statement can be found in the Australian Taxation Office (ATO) profile in your myGov account. This must be a current statement up to the time of application.
  • Graduation Certificate or course completion letter – a copy of your graduation certificate, academic transcript or letter of completion from your Queensland university that demonstrates your area of study, study location and date of graduation.
  • Evidence of working in energy sector or on energy projects – provide letters from employers that evidence at least three years experience in the energy sector or on energy projects. Letters should confirm the project details, dates and duties of the applicant. If letters are not in English, a translated copy must be provided as well.
